This 9"x 12" color & black-and-white 11-pager includes Caddy & Pontiac ambulances, plus a 2-page brochure. It's complete and in good condition, but the stapled fold is almost completely split. It's yours for the postage. I have color scans of the entire document if anyone wants one, at no cost. The real page 2 is a sleeve that holds the two-sided brochure, otherwise blank. I'm at jackramz@att.net.
