What do we call this type of trim and where do you suggest I look to purchase some new pieces?

in the condition that is in i would call it more work. Remove it, light sand with fine paper and buff.

That is an option. Part of the issue is that at some point some of the trim pieces came loose and they were screwed back in place from the front.
But did they use a different screw each time and put it in a different spot each time? There are people the can weld that stuff. But if it's just flat stock might be cheaper to must buy some..
