That is a flat piece of glass, and if you have the old one, almost any auto glass shop can cut you a new one. Once you have the new piece of glass, you can have a vinyl shop make a pattern from the original glass, and the piece of vinyl will be like a negative, with the frosted areas cut out. Then all you need is a friend with a air compressor and a sand blaster. If you don't have a friend with a sand blaster, then you can check with a cemetery monument company to see if they will do it.
I know of a person that broke the original etched glass, and made a replacement, and before he finished working on the car, he broke the second piece of glass. Rumor has it he is a master at etching glass... :D
