'Phog' Allen was a coach at Kansas University and an Osteopathic doctor who treated his players so I don't think this was a tube box. I found a picture of another coach with his 'war bag' on ebay.
 
Definitely not a tube Caddy.
I have a tube caddy stashed someplace, and it's totally different in design. Outside is mostly orange patterned paper, inside is black paper. Also has 2 plastic curtain type tube retainers in the top section.
 
Jeez Friday evening I did the Auction at the Kutztown Radio Show a twice annual event. Guess what I sold for a consigner 15 empty tube caddies for between two and five dollars each. Never thought of them as vintage EMT cases.
